The Clinical Pharmacist is a key member of the healthcare team at Lake Health District, responsible for providing safe, effective, and prompt medication therapy to patients. This role requires a strong foundation in clinical pharmacy practice, with a focus on delivering optimal medication therapy and supporting medication-use systems.
Key Responsibilities- Provide direct patient care in coordination with physicians, nurses, and other hospital personnel.
- Assess patient needs, incorporate age and disease-specific characteristics into drug therapy, and provide individualized pharmaceutical care.
- Participate in quality management initiatives to improve medication-use processes and pharmacy practice.
- Collaborate with interdisciplinary teams to deliver medication therapy and ensure pharmaceutical care programs are integrated throughout the hospital.
- Graduate of an ACPE-accredited College of Pharmacy.
- Licensed (or eligible for licensure) in the state of Oregon.
- Clinical pharmacy experience in a hospital setting preferred.
