As a Care Coordinator, you will play a vital role in assisting patients with their healthcare needs, managing care, and addressing social determinants of health for Medicaid recipients with chronic health conditions.

Key Responsibilities
  • Build and Maintain Relationships: Develop and maintain strong relationships with patients, their families, and healthcare providers to ensure seamless care coordination.
  • Conduct Assessments: Conduct face-to-face assessments to identify patients' medical and social needs, and develop personalized care plans in collaboration with healthcare providers and caregivers.
  • Provide Community Resources: Connect patients with community resources to promote health and well-being, and facilitate access to timely and appropriate care.
  • Schedule Appointments and Transportation: Coordinate appointments and transportation for patients to ensure timely access to medical services.
  • Advocate and Educate: Serve as a patient advocate, providing education and support to patients and their families on healthcare options and community resources.
  • Document and Communicate: Maintain accurate and up-to-date documentation of patient interactions, care plans, and outcomes, and communicate effectively with healthcare providers and stakeholders.
Requirements
  • Experience: 2 years of experience in social services or a related field.
  • Education: Associate's degree required.
  • Language Skills: Bi-lingual Spanish strongly preferred.
