Job Summary

We are seeking a highly skilled Field Nurse Case Manager to join our team in Athens, GA. As a Field Case Manager, you will work in a home-based setting, traveling up to 2 hours one-way to doctors' offices, hospitals, and other locations. Your primary responsibility will be to provide medical case management services for work-related injuries, including assessment, planning, coordination, implementation, and evaluation of injured/disabled individuals.

Key Responsibilities
  • Assess and develop individualized care plans for injured workers
  • Coordinate with insurance carriers, medical providers, attorneys, employers, and employees to ensure seamless care delivery
  • Monitor progress and report results to employers and insurance carriers
  • Collaborate with multidisciplinary teams to achieve optimal patient outcomes
Requirements
  • Current Registered Nurse (RN) license in GA
  • CCM, CRRN, or COHN certification required
  • GA Rehabilitation Supplier certification required
  • Experience in workers compensation case management preferred
  • Excellent organization, communication, and interpersonal skills
