Job Summary

We are seeking a skilled Case Manager to join our team at Avita Community Partners. As a Case Manager, you will provide direct service to our clients by teaching daily living skills and linking them to needed benefits.

Key Responsibilities
  • Provide direct service to clients, including teaching daily living skills and linking them to needed benefits
  • Work collaboratively with clients, family members, and other service providers to develop and implement individualized service plans
  • Conduct regular assessments and evaluations to ensure clients are meeting their goals and objectives
  • Develop and maintain relationships with community resources and service providers to ensure clients have access to necessary services
Requirements

To be successful in this role, you will need:

  • A Bachelor's Degree in Social Work, Psychology, Mental Health, Counseling, or a related field
  • 1 year of experience working in a community mental health setting providing direct services (Level 3)
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
