Job SummaryWe are seeking a highly motivated and compassionate Case Manager to join our team. As a Case Manager, you will provide direct service to our clients by teaching daily living skills and linking them to needed benefits. You will work closely with our clients to develop and implement individualized service plans that promote their independence and well-being.

- 1 year of experience working in a community mental health setting providing direct services (Level 3)
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Flexibility and adaptability in a fast-paced environment
