Nurse Care Coordinator

Mass General Brigham is seeking a skilled Nurse Care Coordinator to facilitate the discharge planning process for patients admitted to Medical, Surgical, Pediatric, or Obstetrical Services. The successful candidate will have a strong background in nursing and excellent communication skills to ensure seamless transitions of care.
Responsibilities:- Coordinate post-acute care services for patients, including hospital outpatient areas, emergency departments, and Henderson Surgical Center.
- Develop and implement discharge plans in collaboration with patients, healthcare teams, insurers, and community-based organizations.
- Utilize high-risk screening criteria to identify patients in need of specialized care coordination services.
- Consult with physicians to ensure medical necessity of admissions and appropriate levels of care.
- Review patients' health records and communicate with healthcare teams to ensure continued appropriate levels of care.
The ideal candidate will possess a strong understanding of healthcare reimbursement, utilization review, quality improvement, and enhanced models of healthcare delivery. Experience in acute care hospital care coordination/discharge planning and knowledge of special needs and behaviors of infant, child, adolescent, adult, and older adult patients are highly desirable.
Requirements:- Graduate of an accredited school of nursing, BSN preferred.
- Current licensure as a Registered Nurse in the Commonwealth of Massachusetts, with a minimum of three years' clinical experience.
- Certifications in Care Management (CCM) or Case Management Administration (ACMA) a plus.
