Key Responsibilities
  • Collaborate with the interdisciplinary team to provide comprehensive biopsychosocial spiritual cultural assessments and develop patient-centered care plans.
  • Anticipate and address patient-specific psychosocial needs, including end-of-life planning, counseling, and crisis interventions.
  • Provide education on end-of-life processes and support patients and families in making informed decisions.
  • Assist with Advance Care Planning and advocate for patients' comfort and dignity.
  • Supervise and educate other social workers as needed.
Requirements
  • Master's degree in social work from an accredited program.
  • Current State Social Work Licensure.
  • Valid motor vehicle operator's license and current insurance.
Work Environment

This role is based in the Support Services Building, with a day rotation schedule. The ideal candidate will possess excellent communication and interpersonal skills, with the ability to work effectively in a team environment.

Compensation and Benefits
