Job Summary

The Social Worker-Case Manager is responsible for assessing the social and emotional needs of patients and their families, coordinating discharge planning, and serving as an advocate and liaison between hospital staff, patients, and the community. This role requires expertise in interviewing techniques, medical terminology, and knowledge of area healthcare facilities and social service agencies.

Key Responsibilities
  • Assess patient and family social and emotional needs
  • Coordinate discharge planning
  • Serve as an advocate and liaison between hospital staff, patients, and the community
  • Develop and implement programs to meet the social and emotional needs of patients and families
Requirements
  • Bachelor's Degree in Social Work from an accredited college or university
  • Minnesota Social Work licensure
  • Wisconsin Background Study upon hire and renewed per Wisconsin state law
Preferred Qualifications
  • Prior experience in a healthcare setting
Knowledge, Skills, and Abilities
  • Ability to integrate social work concepts and functions into the medical setting
  • Knowledge of policies, regulations, and procedures of the hospital
  • Expertise in interviewing techniques
  • Knowledge of medical terminology
  • Ability to make and implement professional judgments
  • Ability to establish and maintain good working relationships with patients, families, public, and coworkers
  • Ability to maintain confidentiality
  • Ability to respond positively to supervision
  • Ability to express ideas clearly in written and verbal forms
  • Ability to perform duties accurately and efficiently while under stress
