About the Role

We are seeking a compassionate and skilled Personal Care Assistant to join our team at the Brandman Centers for Senior Care (BCSC), a Program of All-inclusive Care for the Elderly (PACE). As a Personal Care Assistant, you will play a vital role in providing high-quality, comprehensive healthcare services to our participants.

Key Responsibilities

  • Assist participants with daily living activities, such as bathing, grooming, toileting, and feeding, while maintaining a clean and sanitary environment.
  • Accompany participants to medical appointments and trips to and from the center as needed, ensuring their safety and well-being.
  • Provide translation services to other departments as required, facilitating effective communication and care.
  • Monitor vital signs, such as weight, blood pressure, and pulse, as ordered by the clinic, and maintain accurate records.
  • Manage personal care supplies, including incontinence products, linen, and personal grooming items, ensuring adequate stock and efficient use.
  • Provide laundry services in the PACE Center, adhering to agreed-upon protocols.
  • Lead transitional exercise programs in coordination with the rehabilitation department, promoting physical activity and mobility.
  • Assist with leading activities and the walking group in coordination with the activities department, fostering social interaction and engagement.
  • Monitor participants who need supervision while smoking at the center, ensuring their safety and well-being.
  • Participate in the monitoring of food and fluid intake for specific participants, as identified in their Plan of Care.
  • Assist in the monitoring of participant physical and chemical restraints, adhering to established protocols.
  • Ensure portable meals are sent home with participants as approved by the Interdisciplinary Team.
  • Assist participants upon arrival and departure, helping them prepare for and transition between activities.
  • Immediately report any observed changes in a participant's behavior, health, or mobility to the Center Coordinator and Nurse.
  • Participate on the Interdisciplinary Team as required, communicating effectively with participants, family members, caregivers, staff, and visitors.
  • Monitor safety, infection, and exposure control, being prepared in case of emergency, and adhering to good body mechanics.
  • Complete documentation in the medical record accurately and in a timely manner.
  • Comply with safety policies and procedures, identifying and reporting any potential or actual unsafe acts or conditions to the Center Coordinator and/or Center Manager.
  • Meet or exceed BCSC targets for productivity, quality, and customer service.
  • Continuously seek better ways to deliver services and communicate with participants, promoting a culture of excellence and continuous improvement.

Requirements

  • High school diploma or equivalent required.
  • Certified Nurse Assistant (CNA) certification required.
  • Verification of completion of CPR (BLS) and first aid training required.
  • Valid California driver's license with a good driving record required.
  • TB screening and successful completion of a health examination by a hospital-designated nurse practitioner or physician required.
  • At least one year of experience as a home aide or personal care attendant required.
  • At least one year of experience working with the frail or elderly required.
  • Ability to provide care for the frail elderly, teamwork, and problem-solving skills required.
