- Communicate with Healthcare Teams: Facilitate effective communication among healthcare team members, ensuring that all stakeholders are informed and aligned.
- Support Patient Resources: Connect patients with necessary resources, empowering them to take control of their health.
- Collaborate with Physicians and Healthcare Professionals: Work closely with physicians and other healthcare professionals to ensure comprehensive care.
- Participate in Multidisciplinary Rounds: Engage in multidisciplinary rounds to discuss patient progress and develop effective care plans.
- Document Patient Information: Accurately document patient information in the medical record, ensuring that all relevant details are captured.
Requirements:
- Education: Graduate of an accredited school of nursing or Bachelor's Degree in Social Work.
- Experience: Experience in clinical or acute care settings preferred.
- Certifications/Licenses: LVN/LPN or LBSW licensure in the state of employment required.
Work Schedule and Type:
Full-time position, working 5 days a week with an 8-hour schedule.
