Care Manager I

Job Summary:
The Care Manager I is a vital member of our healthcare team, working in collaboration with patients, families, physicians, and multidisciplinary team members to ensure seamless patient progression through the continuum of care.
This role requires a Social Worker degree and a license in the state of employment, with experience in the clinical or acute care setting preferred.
Key Responsibilities:
- Conduct interviews with patients and families to assess social, emotional, and financial factors impacting health status.
- Develop and monitor patient plans of care to ensure effectiveness and appropriateness of services.
- Coordinate patient care progression throughout the continuum of care, ensuring efficient and cost-effective delivery.
- Serve as a resource, provide support, and advocate on behalf of patients related to treatment decisions and end-of-life issues.
- Closely monitor patient length of stay and collaborate with interdisciplinary team members to remove barriers and expedite discharge.
- Identify and escalate local and system barriers impeding diagnostic or treatment progress and quality and risk issues.
- Work to resolve identified delays to discharge and collaborate with medical staff, nursing staff, and ancillary staff to eliminate barriers to efficient delivery of care.
- Assess needs for discharge planning and continuing care/resource support following discharge.
Requirements:
- Graduate of an accredited school for Social Worker (BSW, MSW, CSW)
- Experience in the clinical or acute care setting preferred
- Social Worker license in the state of employment required
Work Schedule:
Week one: three 12-hour shifts (7a-7p); Week two: three 12-hour shifts (7a-7p) plus an additional 8-hour shift (8a-5p). Totaling 80 hours per pay period.
