About the Role

This position is part of our team in the Emergency Department, requiring a Social Worker degree and a TX License as a Social Worker. The Care Manager I works collaboratively with patients, families, physicians, and multidisciplinary team members to ensure patient progression through the continuum of care and develop a plan of care for each assigned patient from admission through discharge.

Key Responsibilities
  • Conduct interviews with patients and families to gather information about social, emotional, and financial factors impacting health status, both prior to and after discharge.
  • Work with CMII or CMIII to develop and monitor patient plans of care, ensuring effectiveness and appropriateness of services.
  • Coordinate and facilitate patient care progression throughout the continuum of care in an efficient and cost-effective manner.
  • Provide support and act as an advocate on behalf of patients related to treatment decisions and end-of-life issues.
  • Closely monitor patient length of stay and communicate with interdisciplinary team members to remove barriers and expedite discharge.
  • Identify and escalate local and system barriers impeding diagnostic or treatment progress and quality and risk issues as necessary.
  • Collaborate with medical staff, nursing staff, and ancillary staff to eliminate barriers to efficient delivery of care in the appropriate setting.
  • Assess needs for discharge planning and continuing care/resource support following discharge, making recommendations to patients and families regarding post-acute level of care needs and options.
Requirements
  • Graduate of an accredited school for Social Worker (BSW, MSW, CSW).
  • Experience in the clinical or acute care setting preferred.
  • Social Worker license in the state of employment is required (Texas: LBSW, LMSW, or LCSW).
Work Schedule

Week one: three 12-hour shifts (7a-7p); Week two: three 12-hour shifts (7a-7p) plus an additional 8-hour shift (8a-5p), totaling 80 hours per pay period.
