We are seeking a highly skilled Clinical Pharmacist Specialist to join our team at Methodist Hospital. As a key member of our healthcare team, you will be responsible for ensuring the highest quality patient care and treatment outcomes.

Job Summary

This role requires a strong background in pharmacy practice, with a focus on transplant medicine. You will work closely with our multidisciplinary team to develop and implement patient care plans that meet the unique needs of each individual. In addition, you will participate in the evaluation and implementation of clinical programs aimed at improving patient outcomes and streamlining our services.

Responsibilities:
  • Develop and implement comprehensive patient care plans in collaboration with other healthcare professionals.
  • Evaluate and interpret laboratory results, medication lists, and other relevant data to inform treatment decisions.
  • Conduct medication therapy reviews and provide recommendations to optimize patient care.
  • Pursue ongoing education and professional development to stay current on best practices and emerging trends in transplant medicine.
Requirements:
  • Doctor of Pharmacy (PharmD) or Bachelor of Science in Pharmacy degree.
  • A minimum of 5 years of experience in hospital pharmacy, preferably in transplant medicine.
  • BCPS certification or Board of Pharmacy Specialty (BPS) certification in a relevant area.
  • Current Registered Pharmacist license in good standing with the Texas State Board of Pharmacy or equivalent in another state.
