Autumn Care of Norfolk is seeking a skilled and dedicated Interim MDS Coordinator to join our team. As a key member of our healthcare team, you will be responsible for completing the Resident Assessment Instrument (RAI) process in accordance with state and federal laws, as well as our community's policies and goals.
Responsibilities:- Complete the RAI process in a timely and accurate manner
- Collaborate with healthcare team members to ensure comprehensive care
- Develop and implement individualized care plans
- Monitor and evaluate resident outcomes
- Communicate effectively with residents, families, and healthcare team members
- Active, unencumbered Registered Nurse license in the state of practice
- One year of experience in working with the RAI process
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
