At CenterWell Home Health, we're dedicated to providing personalized care to our patients. As a Home Health Speech Language Pathologist, you'll play a vital role in helping our patients recover and regain their independence.

Key Responsibilities:
  • Evaluate and direct speech/language pathology services to patients in their homes or facilities
  • Develop and review treatment plans to ensure optimal patient outcomes
  • Utilize professional skills and judgment to assess and treat disorders of speech, voice, language, hearing, and swallowing
  • Administer and interpret diagnostic tests and therapeutic treatments, including audio logic screening
  • Observe, record, and report changes in patient condition and response to treatment
  • Provide instruction and training to patients in alternative communication systems when necessary
  • Counsel and instruct patients, families, and healthcare staff on speech and language development
  • Maintain accurate documentation and submit timely reports as required
  • Participate in care coordination and discharge planning activities
  • Meet the highest standards of professional conduct and maintain confidentiality
  • Attend and participate in internal staff development programs and continuing education
Requirements:
  • Meet the education and experience requirements for Certification of Clinical Competence in Speech-Language Pathology or Audiology granted by ASHA
  • Minimum of six months experience as a speech therapist/speech-language pathologist
  • Home Health experience a plus
  • Current and unrestricted license
  • Current CPR certification
  • Good organizational and communication skills
  • A valid driver's license, auto insurance, and reliable transportation are required
