  • Contribute to discharge planning and offer patient education along with resource referrals.
  • Analyze patient charts to pinpoint issues related to service delivery, patient outcomes, and regulatory compliance.

Required Qualifications:

  • Possession of a valid Maine state license as a Licensed Social Worker (LSW) or Registered Nurse (RN) is essential.
  • Demonstrated skills in teamwork, communication, and critical thinking are necessary.
