We are seeking a highly motivated and compassionate Patient Care Coordinator to join our team at Henry Street Settlement. As a key member of our healthcare team, you will play a vital role in coordinating patient care services, promoting cancer screening and diagnosis, and supporting vulnerable populations in the Lower East Side community.

About the Role:

The ideal candidate will have excellent communication skills, be able to work collaboratively with healthcare providers and community resources, and possess a strong understanding of social determinants of health (SDOH) and their impact on client wellness. This is an exciting opportunity to make a meaningful difference in the lives of others while working in a dynamic and supportive environment.

Responsibilities:
  • Identify eligible patients for cancer screening and diagnosis services
  • Provide education and support to patients, including obtaining informed consent and managing data collection
  • Assess SDOH and connect patients to appropriate resources
  • Schedule and confirm cancer screening appointments, provide reminders, and ensure follow-through
Requirements:
  • BA/BSW, MA/MSW, MPH, or RN/Nurse Practitioner degree in a related field
  • Excellent communication and interpersonal skills
  • Ability to work collaboratively with healthcare providers and community resources
  • Familiarity with databases and data entry
  • Bilingual in English/Spanish preferred
