About the Role

We are seeking a skilled Licensed Practical Nurse (LPN) to join our Home Health team at Providence. As an LPN, you will play a vital role in delivering high-quality patient care in the comfort of our patients' homes.

Responsibilities
  • Provide skilled nursing care to patients in their homes, focusing on their physical, emotional, and social needs.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Collaborate with other healthcare professionals to ensure comprehensive care and coordination.
  • Conduct thorough assessments, identify patient needs, and develop effective care plans.
  • Administer medications, perform treatments, and provide wound care as needed.
  • Document patient progress, communicate with healthcare teams, and maintain accurate records.
Requirements
  • Graduate of an accredited Licensed Practical/Vocational Nurse program.
  • Current Washington state LPN license.
  • Basic Life Support (BLS) certification.
  • National Provider Identifier (NPI) number.
Preferred Qualifications
  • 1 year of recent hospital, nursing home, or home health experience.
