We are seeking compassionate and skilled Certified Nursing Assistants to join our team at AHN Healthcare@Home. As a Hospice Certified Nursing Assistant, you will play a vital role in providing high-quality patient care and support to individuals in need.
About the Role:
- Provide direct patient care and support to patients in their homes
- Collaborate with our interdisciplinary team to develop and implement patient care plans
- Assist with daily living activities, such as bathing, dressing, and grooming
- Monitor patient vital signs and report any changes to the healthcare team
- Provide emotional support and companionship to patients and their families

Requirements:
- Certified Nursing Assistant Certification in the state you work

- Demonstrated experience with tablets, mobile phones, and EMR software
