Hospice RN Case Manager

As a Hospice RN Case Manager at Gentiva Hospice, you will provide exceptional patient care and serve as the primary liaison between physicians, patients, and the hospice team. This role requires a compassionate and empathetic individual with a strong nursing background and excellent communication skills.
Key Responsibilities:
Deliver direct patient care services and supportive care
Serve as the primary liaison between physicians, patients, and the hospice team
Collaborate with the hospice team to develop and implement patient care plans
Provide education and support to patients and their families
Requirements:
RN license in the state of requested employment or eligibility to obtain
1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology
Current driver's license and automobile insurance
Compassionate and empathetic with a hospice heart
Current CPR certification
