Job Overview

Position Summary:

The Case Planner plays a crucial role in managing case services for a group of challenging youth enrolled in the AOBH Program. This position involves delivering essential services, resources, and counseling while collaborating as a vital member of the AOBH team, all aimed at enhancing the well-being and outcomes for the youth served.

Key Qualifications:

  • Master's degree in Social Work, Criminal Justice, or a related field from an accredited institution, along with experience in working with adolescents.
  • A minimum of two years of experience in foster care, specifically with adolescents, and familiarity with relevant systems such as ACS, OCFS, and CONNECTIONS.
  • Understanding of the importance of precise and timely documentation of service delivery and adherence to best practices in casework.
  • Knowledge of the Integrated Treatment Model and Dialectical Behavior Therapy is essential.
  • Experience in preparing FASP, Permanency Hearing Reports, Transition Plans, and Individual Services Plans is required.
  • Comprehensive understanding of child and adolescent development.
  • Ability to work effectively both as part of a team and independently.
