About the Role:

We are seeking an experienced Licensed Practical Nurse (LPN) to join our team as a Skilled Nursing Care Coordinator. In this role, you will be responsible for providing direct nursing care to our residents while managing the daily activities of certified nursing assistants and resident care associates.

Responsibilities:

  • Direct Nursing Care: Administer medications, perform wound care, and collaborate with the interdisciplinary team to ensure holistic care.
  • Nursing Leadership: Oversee compliance with health-related regulations, maintain effective communication regarding residents' health status, and provide guidance to nursing personnel assigned to the unit.
  • Patient Assessment: Assess changes in residents' status, notify the physician and resident's family or representative, and document accordingly.
  • Medication Administration: Transcribe physician orders to medical records and carry out orders as written.
  • Team Collaboration: Collaborate with other members of the interdisciplinary team as needed to ensure residents' needs are holistically met.

Requirements:

  • Education: Associate's or Bachelor's degree in Nursing
  • Licenses/Certifications: Current unrestricted license as a Licensed Practical Nurse (LPN) in practicing state as well as a current CPR certification
  • Experience: 1+ years of Nursing experience preferred
  • Skills & Abilities:
    • Compassion: Ability to demonstrate compassion when working with residents
