Key Responsibilities:

  • Provide direct care and care coordination for clients, focusing on building independence and empowering them to reach their full potential.
  • Support clients with various needs, including housing, employment, managing finances, health, daily living skills, and socialization.
  • Develop and implement personalized plans to help clients achieve their goals and live independently in the community.
  • Collaborate with other service providers to ensure seamless support and connection to necessary resources.
  • Participate in on-call rotations and be available to meet with clients as needed.

What We Offer:

  • A competitive hourly wage of $16.24, with a $0.25 increase after a successful 90-day probationary period.
  • A full-time position with a typical Monday-to-Friday schedule, with flexibility to accommodate occasional evening or weekend activities.
