Job Summary

Pathways Vermont is seeking a highly skilled and compassionate Assertive Community Treatment (ACT) Team Coordinator to join our dynamic team. As a key member of our interdisciplinary team, you will provide flexible and community-based services to individuals experiencing mental health challenges, histories of homelessness, and substance use issues.

Key Responsibilities
  • Engage in Relationship Building: Develop and maintain strong relationships with clients, providing service from a client-centered, harm reduction, and strengths-based approach.
  • Provide One-to-One Support: Offer counseling and support on life skills, housing concerns, and health maintenance, including medication support, housekeeping, and assisting clients with managing their finances.
  • Assist with Housing Navigation: Help clients navigate the housing path, including viewing units, assistance with moving in, life skill development, and resolving landlord/client disputes.
  • Coordinate Community Services: Coordinate and monitor referrals to community services, advocate for client participation, and manage contacts with outside agencies.
  • Develop Community Resource Knowledge: Develop a working knowledge of relevant community resources and demonstrate a willingness to explore new or unfamiliar resources as necessary.
Qualifications
  • Housing First Framework: Ability to work within a Housing First, client-centered, harm reduction framework.
  • Patience and Compassion: Patience, creativity, flexibility, compassion, and sensitivity to persons with disabilities and other marginalized populations.
  • Understanding of Mental Health Challenges: Good understanding of the issues facing persons with mental health challenges or substance use issues, particularly as related to meeting basic needs of food, clothing, shelter, and advocacy.
  • Problem-Solving Skills: Excellent independent problem-solving skills and demonstrated ability to intuitively approach challenging situations.
  • Valid Driver's License: Valid driver's license, a reliable personal vehicle available for work purposes, and current automobile insurance required.
