Hospice Social Worker Position Overview

Bloom Healthcare is seeking an experienced Social Worker to join our Hospice Team, focusing on delivering compassionate care to patients and their families. This role requires driving city-wide and working approximately 7-10 visits per month.

As a member of our team, you'll have the opportunity to develop and maintain therapeutic relationships with patients and their families, providing essential support and guidance throughout the care journey.

Main Responsibilities:
  • Develop and maintain comprehensive plans of care for patients and their families.
  • Provide supportive counseling and facilitate smooth transitions between care environments.
  • Mobilize community resources to meet patients' and families' needs.
  • Teach patients and their caregivers effective coping skills and strategies.
  • Collaborate with interdisciplinary teams to ensure seamless patient care.

Requirements:

  • Master's degree in social work from an accredited institution.
  • Minimum 1-3 years of experience in palliative care or hospice.
  • Demonstrated ability to assess emotional and social needs of patients and their families.
  • Effective communication and interpersonal skills.
  • Able to collaborate with diverse healthcare professionals.
  • Solution-driven problem-solving skills.
  • Ability to function independently and prioritize tasks effectively.

Compensation:
