Key Responsibilities- Assess and evaluate patients' physical therapy needs and develop personalized plans of care.
- Provide physical therapy services to patients in their homes, including exercises, stretches, and other interventions.
- Collaborate with other healthcare professionals to ensure comprehensive care and coordination.
- Communicate effectively with patients, families, and caregivers to ensure understanding and adherence to treatment plans.
- Document patient progress and outcomes in a timely and accurate manner.
- Master's degree in Physical Therapy from an accredited program.
- Current licensure to practice as a Physical Therapist in the state.
- At least two years of experience as a Physical Therapist, preferably in home health care.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
