About the Role

We are seeking a highly skilled Medical Respite Bridge Nurse to join our team at Central Health. This is a hybrid position that requires living in the greater Austin/Travis County area.

Job Summary

The Medical Respite Bridge Nurse will be part of a collaborative, multidisciplinary team that supports the Respite and Bridge Programs. The ideal candidate will have 2 years of nursing experience in a clinic, hospital or related setting and possess an unrestricted Registered Nurse (RN) License in the State of Texas.

Key Responsibilities
  • Provide nursing support for the service delivery of Respite, Bridge, and Mobile Healthcare Services.
  • Collaborate with the patient's care team to optimize clinical outcomes.
  • Oversee assigned team members and ensure efficient patient flow.
  • Assess physical, social, and emotional needs of patients and families.
  • Serve as an advocate and educate patients on diagnosis, treatments, procedures, and medications.
Required Skills and Qualifications
  • Knowledge of issues faced by individuals experiencing homelessness.
  • Sound-critical thinking and decision-making skills.
  • High level of skill at building relationships and providing excellent customer and patient care.
  • Ability to function as a member of a multidisciplinary team.
  • Strong application of Microsoft office and electronic health records.
Estimated Salary: $65,000 - $85,000 per yearAbout Us
