Clinical Case Manager

As a Case Manager at Lincoln Behavioral Services, you will play a vital role in assisting clients with designing and implementing strategies for obtaining services and supports that are goal-oriented and individualized. This includes assessment, planning, linkage, advocacy, coordination, and monitoring to assist in gaining access to needed health and dental services, financial assistance, housing, employment, education, social services, and other services and natural supports developed through the person-centered planning process.
Key Responsibilities
* Assist clients in developing and implementing individualized plans to achieve their goals
* Coordinate with healthcare providers, social services, and other agencies to ensure clients receive necessary support
* Provide advocacy and support to clients to help them navigate complex systems and access resources
* Monitor client progress and make adjustments to plans as needed
Requirements
* Master's degree from a clinical track (e.g., Social Work, Counseling or Psychology)
* State of Michigan LARA licensing: LLBSW, LBSW, LLMSW, LMSW, LLC, or LPC without current, past, or pending disciplinary action
* Ability to maintain a full clinical caseload
* Ability to work full-time, approximately 40 hours per week
