Job Summary

We are looking for a compassionate and experienced RN to join our team as a Per Diem Home Health Infusion Nurse. In this role, you will be responsible for providing infusion therapy to patients in their homes, working closely with our clinical team to ensure the highest level of care.

Responsibilities
  • Provide infusion therapy to patients in their homes, including venous blood draws, central line blood draws, and IV medication administration.
  • Conduct thorough patient assessments to identify and address any potential health risks or concerns.
  • Collaborate with our clinical team to develop and implement individualized care plans that meet the unique needs of each patient.
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination.
  • Document patient care and progress in a timely and accurate manner.
Requirements
  • Current RN licensure in New York State.
  • Minimum of two years of experience in a clinical setting, with a focus on infusion therapy.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare providers.
  • Ability to work independently and as part of a team, with a focus on providing high-quality patient care.
