Job Summary

As a Per Diem RN Home Health Infusion Nurse, you will be responsible for delivering infusion therapy to patients in their homes. You will work closely with patients, families, and healthcare teams to develop and implement individualized care plans that meet the unique needs of each patient.

Responsibilities
  • Provide infusion therapy to patients in their homes, including venous blood draws, central line blood draws, peripheral line placement, and IV medication administration.
  • Conduct thorough patient assessments to identify and address any potential health risks or concerns.
  • Develop and implement individualized care plans that meet the unique needs of each patient.
  • Collaborate with patients, families, and healthcare teams to ensure seamless care coordination and communication.
  • Document patient care and progress in a timely and accurate manner.
  • Participate in ongoing education and training to stay up-to-date on the latest infusion therapy techniques and best practices.
Requirements
  • Current RN licensure in New York State.
  • Minimum of two years of experience in a clinical setting, with at least one year of experience in infusion therapy.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Ability to work independently and as part of a team, with a strong focus on patient-centered care.
  • Basic Life Support (BLS) certification.
