Job Title: Per Diem RN Home Health Infusion Nurse

VNS Health is seeking a skilled Per Diem RN Home Health Infusion Nurse to join our team in Manhattan, New York.

Job Summary:

We are looking for a compassionate and experienced Registered Nurse to provide high-quality home health infusion care to our patients. As a Per Diem RN Home Health Infusion Nurse, you will be responsible for delivering patient-centered care, educating patients and their families, and collaborating with our clinical team to ensure seamless care transitions.

Responsibilities:
  • Provide infusion therapy services to patients in their homes, including venous blood draws, central line blood draws, peripheral line placement, and IV medication administration.
  • Conduct thorough patient assessments, develop and implement individualized care plans, and evaluate patient progress.
  • Collaborate with primary care physicians and clinical teams to gather patient medical history, medication information, and other relevant data.
  • Educate patients and their families on infusion therapy, medication management, and other relevant health topics.
  • Communicate effectively with patients, families, and healthcare teams to ensure continuity of care.
  • Document patient care and progress in a timely and accurate manner.
Requirements:
  • Current registration to practice as a Registered Professional Nurse in New York State.
  • CPR/BLS certification.
  • Minimum of two years of clinical experience as a Registered Nurse, with one year of experience in infusion therapy or a related specialty.
  • Home care experience in a licensed home care service agency or CHHA setting preferred.
  • Exceptional written and oral communication skills, with strong customer service abilities.
