Alternate Solutions Health Network LLC is seeking a skilled Per Diem Nurse to join our team in Canton, Ohio.

Agency: Mercy Medical Center Home Health and Hospice

We are dedicated to providing exceptional patient care and are seeking a compassionate and skilled Per Diem Nurse to join our team. As a Per Diem Nurse, you will have the opportunity to work autonomously and provide one-on-one patient care, utilizing your clinical skills to provide end-of-life care as directed by an attending physician.

Responsibilities:

  • Plan of Care: Evaluate the treatment and medical condition of the patient according to the plan of care.
  • Minimize Patient Risk: Update all doctor orders in patient chart and administer medication as prescribed by the physician.
  • Continuity of Care: Manage multi-disciplinary care as applicable while promoting continuity of care with appropriate admissions, transfers, and discharges.
  • Family Educator/Advocate: Counsel patient and family on the disease/progression and incorporate patient and family in development of plan of care.
  • Policies: Completes all clinical documentation following agency protocol and Medicare/Federal guidelines.
  • Operations: Oversee and supervise total care of patient provided by nurse aides and LPNs.

Requirements:

  • Registered Nurse with current license in the state of employment.
  • Minimum one-year experience as an RN in an acute care setting is preferred.
  • Hospice experience is desired.
  • Valid driver's license and auto insurance in your name as a driver.
  • Capable of all physical demands.
