Per Diem Home Health Nurse

Alternate Solutions Health Network LLC is seeking a skilled and compassionate Per Diem Home Health Nurse to join our team. As a Per Diem Home Health Nurse, you will provide high-quality patient care in the comfort of their own homes, working closely with our interdisciplinary team to ensure the best possible outcomes.
Job Summary:We are looking for a dedicated and detail-oriented Per Diem Home Health Nurse to provide skilled nursing care to patients in their homes. As a Per Diem Home Health Nurse, you will be responsible for assessing patients' needs, developing and implementing individualized care plans, and providing education and support to patients and their families.
Responsibilities:- Assess Patient Needs: Conduct thorough assessments of patients' physical, emotional, and social needs to identify areas for improvement.
- Develop Care Plans: Create individualized care plans that address patients' unique needs and goals, working closely with the interdisciplinary team to ensure comprehensive care.
- Provide Skilled Nursing Care: Deliver high-quality skilled nursing care to patients in their homes, including wound care, medication management, and education on disease management.
- Collaborate with Interdisciplinary Team: Work closely with the interdisciplinary team, including physicians, therapists, and social workers, to ensure comprehensive care and coordination of services.
- Communicate with Patients and Families: Provide education and support to patients and their families, addressing their concerns and questions in a compassionate and empathetic manner.
- Document Patient Information: Accurately and thoroughly document patient information, including assessments, care plans, and progress notes, to ensure continuity of care.
- Current RN License: Must be a licensed Registered Nurse in the state of employment.
- Experience: Minimum one year of experience as a Registered Nurse in an acute care setting, with a preference for home health experience.
- Valid Driver's License: Must have a valid driver's license and auto insurance in your name as a driver.
- Physical Demands: Must be able to meet all physical demands of the job, including lifting, bending, and standing for long periods.
