Position Summary


The Licensed Practical Nurse (LPN) is tasked with employing the nursing process (assessment, diagnosis, planning, implementation, and evaluation) to deliver personalized nursing care to residents.

Collaborates with residents, families, and interdisciplinary team members, including healthcare providers, to plan, execute, and assess care.

Exhibits proficiency and operates within the full scope of nursing knowledge, adhering to age and population-specific standards relevant to their clinical environment.

Provides care for residents across all phases of preventive care, health maintenance, diagnosis, and treatment. Responsible for care coordination, resident assessments, education, and various nursing interventions.


May assume the role of charge nurse and delegate appropriate tasks to licensed and unlicensed assistive personnel, ensuring alignment with job descriptions, skills, resident care needs, and the competence level of the delegate.

Offers adequate support and guidance to delegates to ensure safe and reliable administration of resident care and resolution of service concerns.

Functions within the scopes and standards of nursing practice as defined by the Nurse Practice Act and Administrative Rules in the state of practice and licensure.

The Sanford Professional Nursing Practice acknowledges the Scope and Standards of Practice and the Code of Ethics for Nurses with Interpretive Statements as established by the American Nurses Association as the foundation for nursing care delivery and professional conduct.


Qualifications


Graduation from a nationally accredited nursing program is preferred, including, but not limited to, Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), and National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA).

A Bachelor’s Degree in nursing is preferred.


Must hold an unencumbered LPN license with the State Board of Nursing where practice occurs and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state.

Must obtain and maintain required department-specific competencies and certifications.
