About the Role

We are seeking a skilled Registered Nurse to join our Hospital at Home team. As a key member of our team, you will provide high-quality patient care in the comfort of patients' homes, working closely with our multidisciplinary team to deliver exceptional outcomes.

Key Responsibilities
  • Provide comprehensive nursing care to patients in their homes, including assessment, planning, implementation, and evaluation of care.
  • Collaborate with our team of clinicians, including Command Center RNs, MDs, and Field Therapists, to ensure seamless care coordination.
  • Develop and implement individualized care plans that address patients' unique needs and goals.
  • Conduct thorough assessments of patients' physical and emotional needs, identifying opportunities for improvement and implementing evidence-based interventions.
  • Communicate effectively with patients, families, and healthcare teams to ensure timely and effective care.
Requirements
  • Associate's Degree in Nursing or equivalent experience.
  • Current Washington or Oregon Registered Nurse License.
  • Basic Life Support (BLS) certification.
  • 2 years of medical-surgical experience in an acute care setting.
Preferred Qualifications
  • Bachelor's Degree in Nursing.
  • 3 years of IV/Infusion skills.
  • Recent Home Health experience.
  • Point of Care experience.
