About the Role

We are seeking a skilled Registered Nurse to join our Hospital at Home team. As a key member of our team, you will provide high-quality patient care in the comfort of their own homes.

Key Responsibilities
  • Provide comprehensive nursing care to patients in their homes, including assessment, planning, implementation, and evaluation of care.
  • Collaborate with a multidisciplinary team, including physicians, therapists, and other healthcare professionals, to ensure seamless care coordination.
  • Develop and implement individualized care plans that address the unique needs and goals of each patient.
  • Conduct regular home visits to assess patient progress, provide education, and address any concerns or issues that may arise.
  • Communicate effectively with patients, families, and healthcare team members to ensure timely and accurate information exchange.
Requirements
  • Current RN licensure in Washington or Oregon
  • 1 year of nursing experience, preferably in home health or hospice care
  • Strong clinical skills and ability to work independently
  • Excellent communication and interpersonal skills
  • Ability to work variable shifts, including evenings and weekends
