Patient Care Advocate

The Heartland Community Health Center is seeking a skilled Patient Care Advocate to join its interdisciplinary medical team. This key role provides critical support to Behavioral Health Consultant (BHC) providers, assisting with direct and indirect patient care.

About the Role
  • Support BHCs with patient care activities, including returning calls, scheduling appointments, and follow-up contacts.
  • Evaluate patients in crisis and refer them to the clinic in collaboration with BHCs.
  • Maintain accurate and up-to-date resource documents for patient, staff, and BHC use.
Responsibilities
  • Returns patient calls and schedules appointments.
  • Folllows up on missed appointments and coordinates limited tasks.
  • Works as an extension of BHC services, reinforcing treatment plans through patient contact.
  • Increases patient understanding of health conditions and motivation to engage in self-management.
  • Performs other related duties as assigned.
Requirements
  • High School graduate or equivalent required.
  • Bachelor's Degree in psychology, social work, or related field preferred and/or experience in related services.
  • Strong verbal communication skills and ability to effectively communicate ideas.
  • Ability to establish and maintain effective relationships.
Working Conditions
  • Potential exposure to infection and contagious diseases.
  • Fast-paced, high-stress environment with frequent interruptions.
  • Requires working indoors in environmentally controlled conditions.

Estimated Salary: $22.50 per hour.
