Job Summary:
As a Hospice Nurse III, Start of Care,.8, you will play a vital role in providing high-quality patient care and support to individuals and their families in the comfort of their own homes. This is an exciting opportunity to join Sutter Care at Home - Outer Bay and make a meaningful difference in the lives of our patients.
Key Responsibilities:
- Assess, coordinate, and deliver nursing care to patients, including direct patient care and patient/family education.
- Develop and implement individualized care plans that incorporate psychological, sociocultural, spiritual, economic, and lifestyle factors.
- Foster collaborative relationships with patients, families, physicians, and other healthcare providers through effective communication.
- Adhere to policies, industry standards, best practices, and applicable laws and regulations to ensure a high-quality patient experience.
- Engage in continuous growth and development in professional nursing practice.
Requirements:
- Graduate of an accredited school of nursing.
- Current RN licensure in California.
- BLS certification.
- Valid automobile insurance and a valid driver's license.
- 5 years of recent relevant experience.
- Proficient knowledge of computer applications, including Microsoft Office Suite and Electronic Health Record systems.
Preferred Qualifications:
- EPIC certification.
- Experience working in home health care or hospice settings.
Working Conditions:
This position requires frequent travel to patient homes and may involve working in a fast-paced environment with multiple priorities.
