We are seeking a highly skilled Clinical Care Manager to join our team at Sutter Visiting Nurse Association and Hospice.

Job Summary:

The Clinical Care Manager will be responsible for supervising and overseeing the planning, organizing, and directing of patient care services, ensuring consistent delivery of high-value quality care to all patient populations served.

Key Responsibilities:

  • Establish patient assignments, staff scheduling, and patient visit volume distributions
  • Assist in meeting financial targets by effectively managing utilization, productivity, personnel, and supplies
  • Support and educate staff in resolving operational issues/problems
  • Collaborate with others to coordinate services, streamline and standardize work flows and procedures

Requirements:

  • Graduate of an accredited school of nursing or physical therapy or occupational therapy
  • RN-Registered Nurse of California or PT-Physical Therapist certification
  • Basic Life Support Healthcare Provider certification
  • Department-SCAH: DL-Valid Drivers License and Auto Insurance
  • 5 years recent relevant experience

Preferred Qualifications:

  • Computer skills, including competency with MS Office suite (Outlook, Word, Excel)
  • Ability to learn and efficiently use electronic medical records systems
  • Excellent communication and interpersonal skills

Working Conditions:

Full-time position, 40 hours per week, Monday - Friday, 8 hours per day
