About the Role

We are seeking a compassionate and skilled LVN to join our hospice care team. As a member of our team, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

Key Responsibilities

  • Provide high-quality patient care in a hospice setting
  • Collaborate with the interdisciplinary team to develop and implement patient care plans
  • Conduct thorough assessments and evaluations to identify patient needs
  • Develop and implement individualized care plans to meet patient needs
  • Provide education and support to patients and their families

Requirements

  • Current California LVN license
  • 6 months of experience as an LVN in a healthcare setting
  • National Provider BLS - American Heart Association within 30 days of hire

Preferred Qualifications

  • Home care or hospice experience
