Field Specialist for Client Monitoring Services

Hourly Compensation: $15/hr

Mileage Reimbursement Available (Must possess a personal vehicle and a Valid Driver's License)

Working Hours: 8am – 5pm

Join our dedicated team of professionals.


Support elderly individuals and adults with disabilities in your community by facilitating access to essential resources and overseeing the provision of authorized services.


This role necessitates travel, as you will conduct approximately 15 home visits daily to clients within the designated area.

Your responsibilities will include evaluating the execution of sanctioned services. Initial training will take place remotely for 1 to 2 days.

Key Responsibilities:


Prepare for client visits by reviewing current Louisiana Plans of Care and other relevant participant data within the OPTS and LOCS Long Term Care Programs.

Conduct routine in-person monitoring visits for participants.

Complete Monitoring Checklists, assess provider service logs, and verify time sheets during home visits.

Document any changes in participant status, outcomes, and address any issues or complaints.

Complete Elder Protective and Adult Protective forms as necessary.

Direct provider-related issues to the Provider Monitoring Unit within one business day.

Report any changes in participant status to the designated Client Assessment Specialist within one business day.

Adhere to complaint procedures, escalating and referring complaints to the appropriate authorities for Protective Services as per policy.

Engage in Continuous Quality Improvement (CQI) initiatives and training.

Education and Experience Requirements

Mandatory:
High School Diploma or GED

Minimum of 6 months experience working with older adults or individuals with disabilities in healthcare-related services.

Valid Louisiana Driver's License

Current Louisiana Automobile Insurance

Preferred:
Associate Degree in Health or Human Services

1 year of experience in healthcare-related services.

Familiarity with healthcare systems, elderly, and disabled adult populations is preferred.

Knowledge of Medicaid regulations is preferred.
