Job Title: Clinic RN

We are seeking a highly skilled and compassionate Registered Nurse to join our team at Southeast Primary Care Partners. As a Clinic RN, you will play a vital role in providing exceptional patient care and support to our patients.

Key Responsibilities:
  • Assist Providers and clinical staff in identifying high-risk patients and implementing band stratification into the electronic medical record.
  • Conduct evaluations and patient education nurse visits for chronic disease states, including clear instruction regarding treatment, medications, self-management goals, and action plans.
  • Triage incoming calls and perform outbound calls regarding band stratification allocation.
  • Assist in reviewing patient supportive care at home, social factors, and concerns, including assessment of durable medical equipment (DME), home health, or other resources that could be beneficial for the patient and their family.
  • Communicate with Hospital and Skilled Nursing facilities regarding inpatients and effectively communicate status to the physician and staff.
  • Assist Providers with communication to the Care Management Team and document in the electronic medical record when appropriate.
  • Participate in daily/weekly huddles and Care Coordination meetings, responsible for updating current patient information and status.
  • Perform clinic patient wound care on stable conditions, assess, and document appropriately.
  • Demonstrate ability to multi-task and work with multidisciplinary teams and individuals.
  • Perform all nursing care duties within the scope of a Licensed Practical Nurse/Registered Nurse, including injections, procedures, phlebotomy, and diagnostic testing.
Requirements:
  • Registered Nurse with a current license in good standing to practice in the state of employment.
  • Current CPR/BLS certification.
  • Ability to work efficiently with electronic medical records.
  • Effective use of proper grammar, spelling, medical terminology.
  • Ability to react calmly and effectively in emergency situations.
  • Two years of related RN experience in a physician's office, clinical, or hospital setting.
  • Current IV certification.
  • Effective listening and communication skills.
  • Infusion experience.
Preferred Qualifications:
  • Three or more years of related RN experience in a physician's office, clinical, or hospital setting.
  • Knowledge of managed care and referral processes.
  • Experience related to patient education.
  • X-ray experience.
  • Proficient knowledge of chronic diseases, especially with COPD/asthma, CHF, IHD, and diabetes.
Physical & Mental Requirements:
  • Ability to lift up to 50 pounds.
  • Ability to push or pull heavy objects using up to 50 pounds of force.
  • Ability to sit for extended periods of time.
  • Ability to stand for extended periods of time.
  • Ability to use fine motor skills to operate office equipment and/or machinery.
  • Ability to receive and comprehend instructions verbally and/or in writing.
  • Ability to use logical reasoning for simple and complex problem-solving.
  • Ability to travel to multiple locations to support business needs as required.
