UnitedHealth Group is seeking a highly skilled Discharge Care Manager RN to join our team. As a key member of our care management team, you will play a critical role in ensuring the seamless transition of patients from acute care settings to their homes or other post-acute care facilities.

Key Responsibilities:

  • Assess and interpret patient needs to identify solutions that promote healthy outcomes
  • Collaborate with healthcare providers and patients to coordinate care post-discharge
  • Develop and implement discharge plans in collaboration with other clinical care team members
  • Identify internal or community-based program support or resources to enhance patient care
  • Coordinate with facility discharge planners to ensure post-hospital services are arranged prior to discharge
  • Document discharge activities and data entry in accordance with standard operating procedures
  • Participate in team meetings, education discussions, and related activities

Requirements:

  • Bachelor of Science in Nursing
  • Current, unrestricted RN license in the state of residence
  • 5+ years of recent experience in the inpatient acute setting
  • 2+ years of experience in discharge planning
  • Proficient in typing skills and software applications, including Microsoft Office
  • Experience working with multiple health insurance products within the insurance industry

Preferred Qualifications:

  • Case Management Certification
  • Compact RN License
  • InterQual/MCG Guidelines or other nationally recognized practice guidelines
  • Problem-solving skills and ability to analyze complex information
  • Excellent verbal and written communication skills
