Key Responsibilities:
- Conduct comprehensive in-home assessments, including medical history reviews, physical exams, and vital sign checks
- Perform evidence-based practice screenings, such as point-of-care testing, to identify potential health risks
- Develop personalized care plans to address patient needs and improve health outcomes
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination
- Travel up to 85% of the time to visit patients in their homes across New Mexico
Requirements:
- Active NP or PA license or ability to obtain by start date
- Active national NP or PA certification or ability to obtain national certification in state of assignment
- Ability to complete physical requirements of the job, including lifting, navigating stairs, and sitting/standing/kneeling
- Access to reliable transportation for home visits
Preferred Qualifications:
- 1+ years of clinical experience in highest level of education, clinical setting
- Experience in gerontology, cardiology, internal medicine, or endocrinology
- Home health care or home visit experience
