Patient Admissions Specialist

KEY RESPONSIBILITIES:
Oversee the intake, admission, and review processes for new patients.
Conduct insurance benefit verifications and communicate findings to patients, their families, and relevant internal teams.
Deliver continuous evaluations of the patient's condition throughout the admission and utilization phases.
Address inquiries regarding the facility within established policy timelines.
Organize and conduct pre-admission assessments, sharing recommendations with patients or their families.
Collaborate with medical and psychiatric staff to ensure suitable recommendations and admissions.
Maintain accurate and ongoing evaluations of the patient's condition during the admissions, intake, and utilization processes.
Educate patients and their families throughout their stay.
Perform continuous assessments of patients' physical, emotional, social, spiritual, and financial needs, implementing crisis intervention and referrals as necessary.
Provide information about healthcare and social resource systems to empower patients and their families to independently access resources.
