Position Overview

As a Senior Case Management Specialist, you will engage in advanced supervisory case management activities, establishing and nurturing long-term relationships with assigned youth, their families, and service providers to address medical, social, educational, and related needs.

Key Responsibilities

The role involves:
  • Conducting thorough case file reviews and developing case plans.
  • Facilitating family involvement and coordinating group service delivery.
  • Setting goals and evaluating outcomes for assigned youth.
  • Overseeing case management and program activities to ensure effective service delivery.
  • Implementing program objectives and procedures while assessing outcome measures.
  • Managing discharge and release processes to ensure seamless service coordination.
  • Supervising the work of junior staff as needed.

Work Environment

This position requires working under minimal supervision, allowing for significant initiative and independent judgment. Candidates should be prepared to work flexible hours, including potential on-call duties.

Training Requirements

Newly hired case managers will be required to complete a pre-service training academy.

Essential Qualifications

Applicants must possess a high school diploma and a minimum of ten years of relevant full-time experience in case management or related fields.

Preferred Qualifications

A Bachelor's or Master's degree in a relevant field is preferred, along with a substantial background in case management or social work.

Skills and Abilities

Successful candidates will demonstrate:
  • Strong knowledge of community resources and child development.
  • Effective communication skills, both written and verbal.
  • Ability to analyze and assess youth needs.
  • Capability to coordinate services and monitor program effectiveness.

Physical Demands

This role may involve physical activities such as lifting, climbing, and restraining youth when necessary.
