About the RoleAs a Hospice RN Case Manager, you will be responsible for delivering high-quality end-of-life care to patients in their homes. Your primary focus will be on developing trusting relationships with patients and their families, while also ensuring that their physical, psychosocial, and environmental needs are met.
Key Responsibilities- Conduct initial nursing evaluations to determine eligibility for hospice services
- Identify and address patient and family needs through ongoing assessments and care planning
- Collaborate with the patient, family, and healthcare team to provide comprehensive care
- Document patient progress and outcomes in a timely and accurate manner
- Current RN licensure in the state of practice
- One year of clinical experience
- Current CPR certification
- Valid driver's license and access to a dependable vehicle
