Addiction Counselor

We are seeking a highly motivated and compassionate Case Manager to join our team. As a Case Manager, you will play a vital role in providing confidential addiction guidance, education, and support to individuals, families, and the community. You will work closely with a multidisciplinary treatment team to ensure post-treatment placement to the most appropriate level of care for each client.

• Develop and implement client discharge/transfer plans to ensure movement to the appropriate levels of care.
• Coordinate ongoing care for client housing and therapeutic needs with the BriteLife Recovery team, substance abuse treatment providers, healthcare providers, community resources, and family members.
• Document all case management service notes, assessments, reassessments, referrals, and follow-up in the client's clinical record.
• Positively promote BriteLife Recovery to prospective clients, employees, and outside referral sources.
• Collaborate with the client to develop a written aftercare plan that is client-centered and recovery-oriented.
• Provide active case consultation with the multidisciplinary team.

• Bachelor's Degree in Behavioral Health Related field preferred.
• Preferred Certified Recovery Specialist or Certified Family Recovery Specialist certificate.
• Minimum of two years of experience working in the substance abuse treatment field.
• Demonstrated computer proficiency.
• Prior experience working at a healthcare setting preferred.
