Pediatric Home Care LPN Job Summary

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team in providing high-quality pediatric home care services in the Metairie, LA area.

This is a full-time position, requiring 12-hour shifts, with opportunities for flexible scheduling and competitive pay rates.

As a Pediatric Home Care LPN, you will be responsible for providing direct nursing care to acutely ill and/or chronically ill patients in their homes, while working closely with our interdisciplinary team to ensure the highest level of patient care.

Key Responsibilities:
  • Provide high-quality clinical services within the scope of practice, as defined by the state laws governing the practice of nursing, in accordance with the plan of care, and in coordination with other members of the health care team.
  • Document patient visits thoroughly and completely per regulatory and payer requirements in the electronic medical record.
  • Actively participate in the agency's care coordination process, including timely reporting patients' needs and changes in condition, attending patient case conference, and communicating effectively with the patient, family, physician, and other clinicians.
Requirements:
  • Current LPN licensure in the state of practice.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
  • Current CPR certification.
