We are seeking a skilled Licensed Practical Nurse (LPN) to provide high-quality care to pediatric patients in their homes. As a Home Care LPN, you will work under the supervision of a Registered Nurse and be responsible for delivering direct nursing care to patients with acute and chronic illnesses.
Key Responsibilities:
- Provide clinical services within the scope of practice, as defined by state laws, in accordance with the plan of care and in coordination with other healthcare team members.
- Document patient visits thoroughly and completely in the electronic medical record, following regulatory and payer requirements.
- Participate in the agency's care coordination process, including timely reporting of patient needs and changes in condition, attending patient case conferences, and communicating effectively with patients, families, physicians, and other clinicians.
- Ensure confidentiality of patient health information and maintain accurate records.
Requirements:
- Current LPN licensure in the state of practice.
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
- Current CPR certification.
- Previous home care experience preferred.
